One size does not fit all. That’s why we offer a range of customized population health strategies, resources, and implementation plans for your practice to succeed in value-based care. You can choose the services that are best for your practice.

 

What should we expect when your team first visits our practice?

The VHS team will meet with the providers and staff in your practice to get an overview of your practice now and your goals for the future. We will then schedule a time to do a deeper dive into your operations so that we may create a customized plan for your transition to value-based care.

 

How often will we work with you?

We will set up the timing that works for your practice in terms of support needed. This may be daily, weekly, bimonthly, or monthly, depending on the stage of the process. Importantly, we are never more than a phone call or video chat away.

 

How many people in our office can work with you?

All of your team members may reach out to us. However, we do recommend that a single point person be selected so that he or she will have an overview of the process.

 

How will you measure success/return on investment (ROI)?

We will measure effectiveness through clinical and health status, patient and provider satisfaction, and financial outcomes. We will set up metrics and schedule regular assessments and reporting to share with staff involved in the care and payers that require such information.

 

How do you bill for your services?

You will be billed monthly for services provided by VHS. You may change the scope of services at any time, in writing, with at least one month’s notice.
